Cityblock, a value-based healthcare provider, has partnered with Sunshine Health to offer comprehensive care to Medicaid members in Central Florida. The partnership aims to deliver seamless primary care and care coordination to high-need, hard-to-reach Medicaid members across 11 counties.

This partnership offers members access to multidisciplinary care teams virtually, at home, or in clinics any day at any time. The goal is to provide whole health support beyond the doctor's office, improving overall health outcomes for Medicaid members. This initiative expands Centene's existing relationship with Cityblock in New York and Ohio.

Dr. Toyin Ajayi, co-founder and CEO of Cityblock, expressed excitement about launching in Florida with Sunshine Health, further developing their national partnership with Centene. “Cityblock has seen incredible growth over the past few years. We are pleased to launch in a new market with a strong Florida leader like Sunshine Health,” said Dr. Ajayi.

Medicaid Members Benefit From Innovative Care Models

This partnership will serve Medicaid members in Volusia, Seminole, Orange, Osceola, Polk, Hernando, Pasco, Hillsborough, Pinellas, Manatee, and Sarasota counties. Sunshine Health's President and CEO, Nathan Landsbaum, highlighted the importance of accessible healthcare. “We are excited to partner with Cityblock to diversify the ways we provide care, engaging members in the modes they most prefer to ultimately improve health outcomes,” said Landsbaum.
