• Provide direct patient care, including assessments, evaluations, and treatments
• Manage LPN staff - perform evaluations and overall supervision
• Administer medications and treatments as prescribed by physicians
• Monitor residents and document observations in medical records
• Collaborate with interdisciplinary healthcare team members to develop and implement individual care plans
• Educate residents and their families on health conditions, treatment options, and preventive care measures
• Assist with health coaching and disease management for residents with chronic conditions
• Perform specialized procedures such as DME fitting, medical imaging, specimen collection processing, and continuous positive airway pressure (CPAP) management
• Provide support and care for residents with special needs or dementia
• Assist with family planning services

• Current FL RN license
• Master’s degree in nursing (MSN) preferred; BSN required
• Minimum 6 years of nursing experience
• Minimum 3 years working with developmentally disabled individuals
• Valid FL Driver's License with acceptable driving record
• Must pass a Level II background check including a drug & TB screening
• Excellent communication and interpersonal skills to effectively interact with patients, families, and staff
• Ability to prioritize tasks and manage time effectively in a fast-paced environment
• Compassionate and empathetic approach to patient care

PHP/FPDC is an intermediate care facility for the intellectually and developmentally disabled. We care for individuals 24/7 in a set of four (4) homes that hold up to 16 individuals.
