Full job description
Responsibilities:
- Provide direct patient care, including assessments, evaluations, and treatments
- Manage LPN staff - perform evaluations and overall supervision
- Administer medications and treatments as prescribed by physicians
- Monitor residents and document observations in medical records
- Collaborate with interdisciplinary healthcare team members to develop and implement individual care plans
- Educate residents and their families on health conditions, treatment options, and preventive care measures
- Assist with health coaching and disease management for residents with chronic conditions
- Perform specialized procedures such as DME fitting, medical imaging, specimen collection processing, and continuous positive airway pressure (CPAP) management
- Provide support and care for residents with special needs or dementia
- Assist with family planning services
- Current FL RN license
- Master’s degree in nursing (MSN) preferred; BSN required
- Minimum 6 years of nursing experience
- Minimum 3 years working with developmentally disabled individuals
- Valid FL Driver's License with acceptable driving record
- Must pass a Level II background check including a drug & TB screening
- Excellent communication and interpersonal skills to effectively interact with patients, families, and staff
- Ability to prioritize tasks and manage time effectively in a fast-paced environment
- Compassionate and empathetic approach to patient care
PHP/FPDC is an intermediate care facility for the intellectually and developmentally disabled. We care for individuals 24/7 in a set of four (4) homes that hold up to 16 individuals.
